Inclusion criteria
Inclusion criteria: 1.Physicians: (1) Hold a practicing (assistant) physician qualification; (2) Have worked in the selected community health service center for at least one year; (3) Work responsibilities involve chronic disease management, including but not limited to hypertension, diabetes, etc.; (4) Agree to participate in the study and sign the informed consent form. 2.Patients: (1) Aged 18 years or older; (2) Clinically diagnosed with at least one chronic non-communicable disease (specifically limited to essential hypertension and type 2 diabetes mellitus); (3) Currently a registered patient at the selected Community Health Center (CHC), or a permanent resident who has received regular follow-up and pharmacotherapy at the center for no less than one year; (4) A permanent resident of study site city during the study period, with no plans for long-term travel outside the city (e.g., returning to hometown, tourism) in the upcoming 3 months.(5) The participant or their accompanying family member possesses proficient Mandarin listening and speaking skills, enabling them to cooperate with the physician for face-to-face interviews and questionnaire surveys. (6) Fully understands the study content, participates voluntarily, and signs the written Informed Consent Form.
Exclusion criteria
Exclusion criteria: 1.Physicians:(1) Clinical practice does not involve the management of non-communicable diseases;(2) Have serious physical illnesses or communication impairments;(3) Are unwilling to participate in the study or refuse to sign the informed consent form. 2.Patients:(1) Currently in the acute complication phase of conditions such as hypertensive crisis or diabetic ketoacidosis, or suffering from other severe physical illnesses requiring urgent hospitalization;(2) Currently participating in other related clinical trials or health management intervention programs;(3) No record of visits to the Community Health Center (CHC) in the past 6 months.(4) Suffering from severe cognitive impairment or other psychiatric disorders affecting communication, rendering them unable to complete effective dialogues and questionnaires, either independently or with assistance.
