Inclusion criteria
Inclusion criteria: 1. Clearly diagnosed with type 2 diabetes and meeting the criteria for high-risk diabetic foot population as defined in the aforementioned "Expert Consensus on Primary Screening and Prevention of Diabetic Foot." 2. Permanently residing within the community covered by the study, expected to have a stable residence in the next year, and having at least one cohabiting family member or guardian who can provide daily care. 3. Aged =18 years, clear consciousness, possessing basic communication and understanding abilities, voluntarily participating in this study and signing a written informed consent form.
Exclusion criteria
Exclusion criteria: 1.Current active diabetic foot ulcer, gangrene, or severe infection (Wagner grade = 2); 2.Comorbid with severe cardiovascular/cerebrovascular diseases, end-stage renal disease, severe liver dysfunction, active malignant tumor, or severe cognitive impairment; 3.Currently participating in other interventional clinical trials that may interfere with the results of this study.
