Aerobic Exercise Intervention Group:Aerobic exercise (5 times per week, 40 minutes each session, including warm-up 5 min, main exercise 30 min, cool-down stretching 5 min
basic phase W1-4: daily steps 4000-5000, advanced phase W5-8: daily steps 5000-6000
auxiliary exercise: pelvic floor training once daily)
Aerobic Plus Resistance Exercise Intervention Group:Aerobic plus resistance exercise (core: 5 sessions of aerobic exercise per week, 2 sessions of resistance exercise per week
auxiliary: pelvic floor training once daily
resistance exercise: 3 sets × 15 repetitions per exercise in basic phase W1-4, 3 sets × 12 repetitions per exercise in advanced phase W5-8)

Inclusion criteria

Inclusion criteria: 1. Aged 50 or older but not exceeding 80 years old, no gender restrictions; 2. Patients histologically confirmed with primary bladder cancer (BC) who are scheduled to undergo transurethral resection of bladder tumor (TURBT) or radical cystectomy (RC). 3. ECOG performance status 0–2, KFS score >= 70 points; 4. Expected survival period >= 1 year; 5. Able to walk independently, with a baseline lifestyle characterized by self-reported moderate-intensity aerobic exercise <150 minutes per week; 6. Voluntarily participate in this study and possess sufficient cognitive and linguistic abilities to understand and sign the informed consent form.

Exclusion criteria

Exclusion criteria: 1. Patients whose primary histological type is metastasis from other cancers, excluding bladder cancer; 2. Patients undergoing palliative surgery or those with cognitive/psychiatric disorders, as well as patients with impaired or severely compromised cardiac function; 3. Unable to move, unable/unwilling to perform personalized exercise plans. Unable to safely exercise from a seated or standing position at home, or has recently fallen or is at high risk of falling. 4. Patients with contraindications to exercise training or cardiopulmonary exercise testing (CPET), or those with severe organ failure, are excluded. Pregnant women are also excluded. 5. Not approved by researchers to participate in the exercise intervention using the Physical Activity Readiness Questionnaire, or unwilling to be randomly assigned to either the supervised exercise training program or the usual care group. 6. Patients who are already participating in other sports rehabilitation programs or who have their own regular exercise regimen.
