Inclusion criteria

Inclusion criteria: 1.Meet the chronic disease criteria specified in the National Application Guidance Manual for International Classification of Diseases and Related Health Problems (ICD-10), and have been diagnosed with at least 2 chronic diseases in a Grade II or higher hospital. 2.Taking =5 types of medications; 3.Conscious and stable vital signs.

Exclusion criteria

Exclusion criteria: 1.Patients with cognitive impairment or a history of mental illness. 2.Patients with severe hearing impairment or language communication barriers who are unable to cooperate with the questionnaire survey.
