Inclusion criteria
Inclusion criteria: Older adults aged 60 years and above who are permanent residents of Shenzhen. Inclusion criteria (meeting any one of the following is sufficient for enrollment): 1. Confirmed diagnosis of at least one chronic disease: hypertension, diabetes mellitus, hyperlipidemia, or cardio-cerebrovascular disease; 2. Mild cognitive impairment (meeting the cutoff scores of MoCA, AD8, or MMSE); 3. Mild physical impairment (normal score on the Activities of Daily Living (ADL) scale and a Short Physical Performance Battery (SPPB) score of 7–10 points).
Exclusion criteria
Exclusion criteria: 1. Confirmed diagnosis of dementia; 2. Unable to cooperate with the investigation due to deafness, blindness, aphonia, etc.; 3. Having medical conditions that make exercise unsafe or being unable to exercise (e.g., depression, uncontrolled hypertension, uncontrolled hyperglycemia, and other neurological diseases); 4. Currently receiving physical or cognitive function treatment or rehabilitation in medical institutions; 5. Planning to move or stay away for a long time within one year; 6. Participating in other trials that may affect the present study.
