Postoperative pain in children undergoing laparoscopic orchiopexy in the lower abdomen

Interventions

Fascial set:Bilateral posterior quadratus lumbus block (QLB2) was performed under ultrasound guidance after induction of anesthesia and prior to surgery
Intramuscular set:After induction of anesthesia before the operation began Sonic-guided bilateral quadratus lumbosi intramuscular block (QLBi) was administered with the same amount of local anesthesia
Control group:The children in the control group did not receive nerve block after general anesthesia.

Inclusion criteria

Inclusion criteria: 1. A total of 210 pediatric patients who underwent laparoscopic abdominal surgery in our hospital from September 2022 to May 2022 were included; 2. Children aged 3 to 7 years, with ASAI-II grade, who underwent postoperative extubation.

Exclusion criteria

Exclusion criteria: 1. Those who refuse to participate or cannot cooperate; 2. Those who temporarily change the operation method due to the need of operation; 3. Allergic to ropivacaine; 4. Severe respiratory, circulatory, liver and kidney function abnormalities; 5. Puncture site infection; 6. Patients with a history of chronic pain who have used opioids for more than 6 months; 7. Coagulation function is obviously abnormal; 8. Ultrasound showed unclear anatomical structure.

The proportion of remedial analgesia within 24h after operation was compared among the three groups;

Secondary

MeasureTime frame
Observe the time of the first postoperative remedial analgesia of the children (real-time assessment of the FLACC score of the children =3 points after the surgery according to the feedback of the parents, and the time of the first remedial drugs).;the FLACC and facial pain scores of the children in the three groups at the time of awakening, 1h, 4h, 6h, 12h and 24h after operation;The types and dosages of remedial analgesia in the three groups were also recorded;the occurrence of QLB related complications such as hematoma at the puncture site, infection, peripheral organ injury, local anesthetic toxicity, lower limb muscle strength loss, and hypotension were recorded;Operation time in ultrasound guided block was recorded;
