Inclusion criteria

Inclusion criteria: Chronic disease patients who were included in the mobile clinic health record management system for remote mountainous areas in Yongning Town and Dazhuangke Township from January to June 2024. 1. The selected chronic disease patients meet the diagnostic criteria specified in Analysis and Evaluation of the Current Status of Clinical Practice Guidelines in Primary Care in China, and suffer from one or more of the following chronic diseases: hypertension, diabetes mellitus, dyslipidemia, and chronic kidney disease. 2. Patients are capable of normal verbal communication. 3. Patients can understand and answer the questionnaire content either independently or with the assistance of auxiliary tools such as drawings and videos. 4. Patients voluntarily participate in the study and sign the informed consent form.

Exclusion criteria

Exclusion criteria: 1. Patients unable to self-manage their medications. 2. Patients with moderate or severe dementia (Mini-Mental State Examination (MMSE) score = 20 points). 3. Patients with severe mental and behavioral disorders. 4. Patients complicated with acute severe heart, liver or kidney diseases caused by other types of trauma. 5. Patients who are unable to cooperate in completing the questionnaire. 6. Patients who are currently participating in other research projects.
