Inclusion criteria
Inclusion criteria: Patient inclusion criteria: (1) Met the diagnostic criteria for acute ischemic stroke according to the Chinese Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke; (2) aged >=18 years; (3) first-ever stroke confirmed by neuroimaging; (4) conscious and able to cooperate with questionnaires and relevant assessments; (5) both the patient and the primary caregiver provided written informed consent and participated voluntarily. Caregiver inclusion criteria: (1) aged >=18 years with full civil capacity; (2) identified by the patient as the primary family caregiver; (3) able to communicate effectively and complete the questionnaire; (4) willing and able to complete the 30-day post-discharge follow-up; (5) participated voluntarily and provided written informed consent.
Exclusion criteria
Exclusion criteria: Patient exclusion criteria: (1) Severe dysfunction of vital organs (e.g., heart, liver, or kidney); (2) pre-existing cognitive impairment or psychiatric disorders; (3) lost to follow-up or refused follow-up. Caregiver exclusion criteria: (1) Significant cognitive or communication impairment, or severe psychiatric illness; (2) not the primary caregiver; (3) unable to complete follow-up as scheduled or lacked stable contact information; (4) severe physical illness or other conditions making participation inappropriate; (5) did not provide informed consent or withdrew informed consent.
