Inclusion criteria
Inclusion criteria: 1. Informed Consent: The patient or their legal representative voluntarily signs the informed consent form and is willing to cooperate with clinical examinations, sample collection, and long-term follow-up. 2. Meet the typical age characteristics for various diseases, generally 18-80 years old. 3. Diagnostic criteria for various diseases: (1) Benign Paroxysmal Positional Vertigo (BPPV): Diagnosis conforms to the "Barany Society BPPV Diagnostic Criteria": short-term vertigo (=2 times within the past 6 months). Positional tests and vestibular function examinations can be performed. (2) Ménière's Disease (MD): Diagnosis conforms to the "Barany Society MD Diagnostic Criteria": at least 2 episodes of persistent vertigo (each >=20 minutes); fluctuating hearing loss (pure tone audiometry showing low-frequency or full-frequency hearing loss); tinnitus and/or a feeling of ear fullness. (3) Vestibular Neuritis (VN): Diagnosis conforms to acute onset, persistent vertigo >=24 hours, accompanied by nausea and vomiting, without hearing loss or with mild hearing loss; cold and hot tests or video head impulse test (vHIT) indicating unilateral vestibular dysfunction; central lesions excluded (e.g., negative brain MRI). (4) Bilateral Vestibulopathy: Diagnosis conforms to chronic progressive balance disorder, unsteady gait, worsened in darkness; bilateral vestibular function tests (e.g., vestibular-evoked myogenic potentials, vHIT) showing bilateral dysfunction; other causes excluded (e.g., drug toxicity, hereditary diseases). (5) Age-related Vestibular Dysfunction: Diagnosis conforms to age >=60 years; chronic balance disorder, with or without vertigo, duration >=3 months; vestibular function tests showing bilateral or unilateral dysfunction, with other clear causes excluded (e.g., cerebrovascular disease, tumors).
Exclusion criteria
Exclusion criteria: 1. Severe systemic diseases: Diseases such as hepatic or renal insufficiency, severe cardiovascular diseases, or malignant tumors that may affect study outcomes or prognosis. 2. Causes of central vertigo: Etiologies including stroke, multiple sclerosis, intracranial tumors, etc. 3. Recent medication effects: Use of immunosuppressants, glucocorticoids, antiviral drugs (only for vestibular neuritis [VN] patients), or metabolism-affecting drugs (e.g., hypoglycemic agents, lipid-lowering agents) within the past 1 month. 4. Pregnant or lactating women. 5. Cognitive impairment or mental disorders: Inability to cooperate with examinations and follow-up. (1) BPPV: Complicated by severe cervical spondylosis or a history of neck trauma (within the past 3 months). (2) MD: Concurrent severe autoimmune diseases (e.g., systemic lupus erythematosus, rheumatoid arthritis). (3) VN: Concurrent herpes zoster virus infection, HIV infection, or a history of other confirmed viral infections. (4) Bilateral vestibular disease / Senile vestibular disease: Concurrent diabetes, thyroid dysfunction, or severe vitamin D deficiency (<10 ng/mL).
