Inclusion criteria
Inclusion criteria: 1. Age >60 years old; 2. The types of chronic diseases included hypertension, diabetes, chronic heart failure, chronic obstructive pulmonary disease; 3. No other critical and severe complications; 4. The patient was conscious and had no speech dysfunction, and was able to cooperate to complete the study.
Exclusion criteria
Exclusion criteria: Patients in the terminal stage, those with severe dementia, critically ill patients, completely disabled individuals, and those who request to withdraw.
