Inclusion criteria
Inclusion criteria: 1. Age >=18 years, diagnosed with CHF according to heart failure guidelines, and classified as New York Heart Association (NYHA) functional class II or III; 2. In a clinically stable phase after treatment, and identified as frail according to the Fried frailty phenotype (meeting >=3 criteria); 3. Physically able to independently participate in Tai Chi exercise, without musculoskeletal disorders affecting joint mobility; 4. Owning a smartphone.
Exclusion criteria
Exclusion criteria: 1. Scheduled for procedures such as coronary artery bypass grafting (CABG) surgery, transcatheter aortic valve implantation (TAVI), and/or heart transplantation within the next 3 months; 2. Presence of severe comorbid diseases in other organs, such as cancer, liver or renal failure; 3. Inability or unwillingness to use remote monitoring devices (e.g., severe dependence in activities of daily living (ADL), dementia, lack of communication ability); 4. Presence of severe musculoskeletal disorders, osteoporosis, or malnutrition; 5. Planned admission to a nursing home or other care institution after discharge; 6. Regular exercisers who have practiced Tai Chi or engaged in other forms of structured exercise within the past six months; 7. Current participation in similar or related research studies.
