Inclusion criteria
Inclusion criteria: Patients 1. Clinically diagnosed with coronary heart disease and ischemic stroke, either simultaneously or sequentially. 2. Activities of Daily Living (ADL) score = 18 years, with informed consent and voluntary participation in this study. Caregivers 1. Primary caregiver providing daily care for the patient. 2. Able to accompany the patient to hospital visits or participate in telephone interviews. 3. No severe mental or cognitive disorders, able to cooperate and complete the questionnaire. 4. Aged >= 18 years, with informed consent and voluntary participation in this study.
Exclusion criteria
Exclusion criteria: Patients 1. Severe language or hearing impairment that affects questionnaire completion. 2. Complicated with advanced malignant tumor, end-stage severe heart failure, or other severe conditions. Caregivers 1. Severe physical disease (e.g., malignant tumor) or mental illness. 2. Severe language or hearing impairment that affects questionnaire completion.
