Inclusion criteria

Inclusion criteria: 1.Age >= 50 years; 2.Current residence in either a private home or an officially licensed nursing facility (including care homes or skilled-nursing institutions) in Shanghai; 3.Fulfillment of at least one, or both, of the aforementioned diagnostic definitions for disability or dementia; 4.Provision of informed consent by the participant or a legally authorized representative, with the capacity to complete a questionnaire and interview within 45 min.

Exclusion criteria

Exclusion criteria: 1.Acute medical event within the preceding 30 days (e.g., acute coronary syndrome, stroke, hip fracture, severe infection); 2.Life expectancy < 6 months due to terminal illness (e.g., advanced malignancy, severe heart failure, respiratory failure); 3.Concurrent total deafness and blindness precluding valid cognitive or sensory assessment; 4.Persistent coma or severe delirium rendering interview participation impossible; 5.History of psychotic disorder or current uncontrolled schizophrenia, schizoaffective disorder, or bipolar disorder; 6.Refusal to provide informed consent or participation in a similar interventional study within the past 30 days.
