Intervention group:1. Structural Empowerment Tools: PICU Transitional Period Family Empowerment Manual, Transitional Care Diary
2. Phased and Standardized Nursing Intervention Process: Conduct joint pre-visits and provide pre-training on core skills for parents before the patient is transferred out of the PICU
assign a liaison nurse to accompany the transfer and use a standardized handover checklist during the transfer out of the PICU
carry out intensified follow-up visits on the day of transfer to the general ward.
Control group:1. Before transfer: The attending physician shall conduct condition notification for parents, covering the explanation of the prognosis of severe pediatric pneumonia, basic medication gu

Inclusion criteria
Inclusion criteria: Inclusion criteria for parents: 1.Voluntarily participated in this study and gave informed consent; 2. Had good language communication and comprehension abilities; 3. Being the primary caregiver of a child hospitalized for severe pneumonia in the PICU and an immediate family member.
Exclusion criteria
Exclusion criteria: Exclusion criteria for parents: 1. Mental disorders or intellectual disabilities; 2. Caregivers employed by the families of children hospitalized for severe pneumonia in the PICU;3.Experienced traumatic life events within the past three months.
