Inclusion criteria

Inclusion criteria: 1. Meets the diagnostic criteria for stroke as stipulated in "Key Points for Diagnosis of Major Cerebrovascular Diseases in China 2019"; 2. Age between 60 and 80 years old; 3. Admission time >= 24 hours; 4. Clear consciousness and able to communicate normally, with a MoCA score >= 26 points for inpatients; 5. Barthel Index >= 60 points (indicating that the patient's self-care ability is below mild dependence and can basically take care of themselves in daily life); 6. OFI-8 score >= 4 points for stroke patients; 7. The patient or their family members have basic literacy skills and can use the WeChat APP with the help of the nurse, and have a fixed family member for care after discharge; 8. Informed consent and voluntary participation.

Exclusion criteria

Exclusion criteria: 1. Patients with severe organ dysfunction were excluded. 2. Patients with a history of psychiatric disorders or malignant tumors were excluded.
