Inclusion criteria
Inclusion criteria: 1. Age >= 18 years; 2. Planning to undergo a kidney transplant; 3. Able to attend follow-ups 1-12 months after surgery and able to use WeChat/smartphone (or willing and able to have family members upload blood glucose data on their behalf).
Exclusion criteria
Exclusion criteria: 1. Unable to use a mobile phone or refuse to participate in remote monitoring; 2. Patients with severe heart, liver, or psychiatric diseases, or those expected to be unable to complete follow-up during the study period; 3. Currently hospitalized, undergoing short-term treatments that affect blood sugar (such as short-term high-dose systemic corticosteroid therapy), and expected not to stabilize in the short term; 4. Known pregnancy or other major diseases that would hinder completion of the study or assessment; 5. Any other conditions that may make the patient unsuitable for participation in this study, or situations where participation may pose additional risks to the patient.
