Inclusion criteria

Inclusion criteria: 1. Meet the clinical diagnostic criteria for COPD: Research subjects must conform to the diagnostic criteria for COPD specified in the Guidelines for the Diagnosis and Treatment of Chronic Obstructive Pulmonary Disease (2021 Revised Edition) [1]. In addition, the disease severity should be classified in accordance with the grading standards of the 2025 Global Strategy for the Diagnosis, Management, and Prevention of Chronic Obstructive Lung Disease (GOLD) [5]. Meanwhile, subjects must satisfy the inclusion and exclusion criteria of this study. 2. Age >= 60 years old. 3. Admitted to hospital with a diagnosis of acute exacerbation of chronic obstructive pulmonary disease (AECOPD). 4. Adequate walking and independent mobility ability: Capable of walking independently without external support;or able to maintain balance during the test even if a walking cane is used in daily life. 5. Potential for rehabilitation (with underlying kinesiophobia): A score of >= 37 points (indicating moderate fear) on the Tampa Scale for Kinesiophobia (TSK) in the baseline assessment. 6. Signed the informed consent form and willing to participate in follow-up visits.

Exclusion criteria

Exclusion criteria: 1.Severe and uncontrolled cardiovascular diseases: Unstable angina pectoris, myocardial infarction occurring within the past 3 months, severe arrhythmia, or uncontrolled hypertension. 2. Mobility limitations caused by non-cardiopulmonary factors (orthopedic/neurological): Such as severe knee/hip osteoarthritis, or rheumatism and rheumatoid diseases that cause significant pain or limitation of joint mobility. A history of post-stroke hemiplegia, Parkinson's disease, or other neuromuscular disorders that severely affect gait and balance. 3. Continuous dependence on walking aids or other forms of heavy reliance: Severe dependence on wheelchairs or walking aids. 4. Cognitive or mental disorders: Inability to understand research instructions. 5. New major changes in the patient’s condition during the study period (for withdrawn or excluded subjects): For example, the occurrence of severe complications requiring transfer to the intensive care unit (ICU) for treatment, new-onset acute myocardial infarction, or pulmonary embolism during the observation period.
