Inclusion criteria
Inclusion criteria: 1. Be at least 18 years old; 2. Previously or currently hospitalized and diagnosed with coronary heart disease; 3.>=3 treatable risk factors; 4. Be a smartphone or wechat user with an available Internet connection; 5. Be a user of the "Yue Jiankang" smart Hospital Cloud platform; 6. Sign the informed consent form.
Exclusion criteria
Exclusion criteria: 1.Patients with coronary heart disease with less than 3 risk factors; 2. Pregnant women; 3. The life expectancy of patients with malignant tumors or severe heart failure is less than one year; 4. Refuse to use social media or sign informed consent; 5. There is cognitive dysfunction or accompanied by mental illness.
