Inclusion criteria
Inclusion criteria: (1) Age >= 60 years old; (2) Suffering from 2 or more chronic diseases simultaneously. All patients meet the criteria for chronic diseases according to the International Classification of Diseases (ICD-10), including 9 common diseases: hypertension, coronary heart disease, cerebrovascular disease, diabetes, chronic obstructive pulmonary disease, malignant tumor, senile osteoarthritis, chronic gastrointestinal disease, and chronic heart failure; (3) Clear consciousness and normal cognitive function; (4) Stable vital signs; (5) Patients with certain reading abilities who voluntarily participated in this study.
Exclusion criteria
Exclusion criteria: (1) People with unclear thinking, dementia, and various mental illnesses; (2) Persons who are bedridden and disabled or terminally ill; (3) Patients with acute exacerbations of chronic diseases or those who have suddenly developed other acute medical conditions. (4)Incomprehensible language expression or language communication disorders, hearing problems, and cognitive impairments.
