Interventions

TCM Constitution-Based Precision AI-HEALS Intervention Group:On the basis of the universal module, personalized AI-HEALS interventions integrated with TCM constitution identification are implemented.
Universal AI-HEALS Intervention Group:The universal AI-HEALS intervention group receives MTM-based AI-HEALS interventions on the basis of routine chronic disease management.
Control Group (Routine Management Group):Routine health education, medication guidance, and regular monitoring of blood pressure, blood glucose, and blood lipids provided by front-line doctor

Inclusion criteria

Inclusion criteria: 1. Age >= 18 years old; 2. Meets the diagnostic criteria for at least two of the following diseases: hypertension, type 2 diabetes, hyperlipidemia; 3. Proficient in using a smartphone and able to operate applications such as WeChat; 4. Able to take care of oneself, with clear thinking ability and coherent speech; 5. Informed consent obtained from the patient, who voluntarily agrees to participate in the study.

Exclusion criteria

Exclusion criteria: 1. Severe dysfunction of vital organs such as the heart, liver, or kidneys; 2. Received radiotherapy or chemotherapy within the past six months; 3. Currently participating in other clinical studies; 4. Women who are pregnant or breastfeeding.
