Interventions

intervention group:Continuous intravenous infusion of esketamine dilution (concentration: 2.0 mg/mL). Loading dose: administered at a rate of 0.1 mg/kg/h over the initial 10 minutes of infusion
maintenance dose: followed by an infusion rate of 0.2 mg/kg/h until the end of surgery. The total infusion duration should be at least 60 minutes.
control group:An equal volume of normal saline is administered at the same rate and for the same duration.

65 Years to 90 Years

Inclusion criteria

Inclusion criteria: 1. Age >= 65 years, gender is not limited. 2. patients who are to undergo elective primary unilateral total hip arthroplasty under lumbar anaesthesia. 3. American Society of Anaesthesiologists (ASA) class II or III. 4. Written informed consent signed by the patient or his/her legal representative. 5. No contraindication to intrathecal anaesthesia.

Exclusion criteria

Exclusion criteria: 1. Severe cognitive impairment assessed preoperatively: score on the Brief Mental Status Examination (MMSE), illiterate = 160 mmHg and/or diastolic blood pressure >= 100 mmHg at rest despite antihypertensive medication. 7. History of elevated intracranial pressure, or history of clinically confirmed acute cerebrovascular accident within the last 6 months.
