Inclusion criteria
Inclusion criteria: 1. Patients: (1) met the diagnostic criteria of China Guidelines for Diagnosis and Treatment of Heart Failure 2024, clinically diagnosed with chronic heart failure; (2) aged 18 years; (3) can complete the questionnaire in written or oral form; (4) gave informed consent and voluntary participation. 2.Primary caregivers: (1) aged 18, with normal communication; (2) takes care of patients for 4h a day, and still undertakes the main care work after discharge; (3) has good communication and understanding skills and volunteered to participate in this study.(4) Can understand what the questionnaire says.
Exclusion criteria
Exclusion criteria: 1.Patients: (1) has major physical diseases, such as cancer, vital organ failure; (2) has a history of psychological disorders or mental diseases; (3) after heart transplantation; (4) recent major life events. 2. Main caregivers: (1) severe mental disorders; (2) receiving care remuneration; (3) with severe intellectual, auditory, visual and language communication disorders.
