Inclusion criteria

Inclusion criteria: 1.Age >= 65 years old. 2.Patients diagnosed by a physician with three or more chronic diseases, with a duration of at least 1 year for each diagnosis. 3.Taking five or more types of medications. 4.Patients who are living at home after hospital discharge. 5.Have provided informed consent and are willing to participate in this study voluntarily.

Exclusion criteria

Exclusion criteria: 1.Combined speech and hearing impairments; 2.Patients with severe cognitive impairments; 3.Participants in other medication-related studies; 4.Terminally ill patients (expected survival time less than 3 months).
