Inclusion criteria

Inclusion criteria: 1.Age >= 18 years; 2.Diagnosed with at least two of the following conditions: hypertension, type 2 diabetes, or hyperlipidemia; 3.Proficient in using smartphones and capable of operating applications such as WeChat; 4.Able to perform activities of daily living independently, with intact cognitive function and clear verbal communication; 5.Provided informed consent and voluntarily agreed to participate in the study.

Exclusion criteria

Exclusion criteria: 1.Presence of severe dysfunction in major organs such as the heart, liver, or kidneys; 2.Received radiotherapy or chemotherapy within the past six months; 3.Currently participating in other clinical studies. 4.Pregnant or lactating women;
