Inclusion criteria

Inclusion criteria: Inclusion criteria of stroke patients: 1. patients met the diagnostic criteria of stroke in "The Diagnostic points of Major Cerebrovascular Diseases in China 2019" [80], and stroke was diagnosed by brain CT or MRI; 2. age >=18 years old; 3.stable condition with certain self-care ability (modified Barthel index score >60 points [81]); 4. normal cognition (MMSE score >=17 points [82]), no obvious communication disorder (Token test scale score >=17 points [83]); 5.Informed consent and voluntary participation in the study; 6. Able to use wechat software on smart phones. Inclusion criteria of caregivers of stroke patients: 1. Informal caregivers (such as spouses, children, etc.) who undertake the main care tasks of stroke patients;2. age >=18 years old; 3. normal cognitive function (MMSE score >=17 points) and able to answer questions correctly and clearly (Token test scale score >=17 points); 4. bear the main care responsibility for patients, and take care of patients for more than 4 hours per day; 5. informed consent and voluntary participation; ? Able to use wechat software on smart phones.

Exclusion criteria

Exclusion criteria: Patinets' exclusion criteria: 1.Patients with other critical diseases, such as malignant tumors, heart failure, renal failure, respiratory failure, severe trauma and other critical diseases; 2. Participating in other investigators. Caregivers' exclusion criteria: unable to cooperate with the survey due to personal reasons.
