Inclusion criteria

Inclusion criteria: 1. Voluntary participation in the study; 2. The patient has been diagnosed with CKD stage 3-4 in accordance with the K/DOQI guidelines formulated by the American Kidney Disease Foundation. And was diagnosed with chronic heart failure according to the 2018 edition of the Chinese Guidelines for the Diagnosis and Treatment of Heart Failure. 3. >=18 years of age; 4. Clear consciousness, good cognitive ability, and a need to acquire correct medical knowledge.

Exclusion criteria

Exclusion criteria: 1. Currently suffering from mental disorders, cognitive impairments, Alzheimer's disease, or other cognitive-affecting conditions, or unable to cooperate for other reasons; 2. Those with severe visual and hearing impairment at present.
