Inclusion criteria
Inclusion criteria: 1. Age >= 18 years; 2. Diagnosed clinically and confirmed by imaging as acute ischemic stroke or transient ischemic attack (TIA), meeting the criteria outlined in the "Chinese Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke (2018)"; 3. Modified Rankin Scale (mRS) score <= 2 at enrollment; 4. Patient is proficient in using a smartphone and WeChat; 5. Voluntarily participates in the study and provides signed informed consent.
Exclusion criteria
Exclusion criteria: 1. Patients with other serious comorbidities, such as malignant tumors or severe cardiac, hepatic, or renal insufficiency; 2. Patients with psychiatric or psychological disorders; 3. Patients with hemiplegia, mobility impairment, or inability to perform activities of daily living independently; 4. Patients residing in areas with inadequate internet access.
