Inclusion criteria

Inclusion criteria: Part 1 1. Age >= 60 years; 2. Suffering from multiple chronic diseases meeting ICD-10 criteria, including the following common conditions: hypertension, coronary heart disease, cerebrovascular disease, diabetes, chronic obstructive pulmonary disease, malignant neoplasm, osteoarthritis, chronic gastrointestinal disease, chronic heart failure; 3. Conscious with normal cognitive function, capable of normal communication; 4. Vital signs stable; 5. Possesses basic reading ability and has provided informed consent for this study. Part 2 1. Age >= 60 years; 2. Suffering from multiple chronic diseases, all patients meet ICD-10 criteria, with the following common conditions: hypertension, coronary heart disease, cerebrovascular disease, diabetes, chronic obstructive pulmonary disease, malignant neoplasm, osteoarthritis, chronic gastrointestinal disease, chronic heart failure. 3. Exclusion criteria

Exclusion criteria: Part 1: 1. Dementia, confused thinking, various psychiatric disorders; 2. Impaired language expression or communication difficulties, hearing problems, cognitive impairment. Part 2: 1. Dementia, confused thinking, various psychiatric disorders; 2. Disabled and bedridden patients or end-of-life patients; 3. Patients in acute exacerbation of chronic disease or with sudden other acute conditions; 4. Impaired language expression or communication difficulties, hearing problems, cognitive impairment.
