Inclusion criteria

Inclusion criteria: 1. Age >18 years and = 32.5 kg/m^2; (2) BMI >= 27.5 kg/m^2 but =90 cm, female waist circumference >= 85 cm, with imaging indicating central obesity; after MDT assessment and ethical approval, the surgical recommendation level can be adjusted accordingly; (4) Patients with 25 kg/m^2 <=BMI < 27.5 kg/m^2 and T2DM who have difficulty controlling blood glucose through lifestyle changes and medication and have obesity-related comorbidities; their insulin secretory function should be evaluated, and surgery can be performed after MDT assessment and ethical approval. 3. No major organ dysfunction, no neuropsychiatric disorders, and no other harmful habits. 4. Informed consent obtained with good compliance, and able to cooperate and complete the study.

Exclusion criteria

Exclusion criteria: 1. Preoperative diagnosis of gastroesophageal reflux disease (GERD); 2. Use of anti-reflux medications (e.g., proton pump inhibitors [PPIs], H2-receptor antagonists) within 1 month prior to surgery; 3. History of anti-reflux surgery or upper abdominal surgery; 4. Pregnancy or lactation; 5. Intraoperative findings of hiatal hernia or other pathologies requiring intervention; 6. Severe cardiocerebrovascular diseases or major organ dysfunction contraindicating surgery or general anesthesia.
