Inclusion criteria
Inclusion criteria: 1.Age >=18 years; 2.Have one or more chronic diseases that qualify for the International Systematic Classification of Diseases (ICD-10); 3.Able to communicate with medical staff in Mandarin; 4.Be able to use the system proficiently after receiving instruction; 5.No mobility impairment;
Exclusion criteria
Exclusion criteria: 1.Other trials were conducted within 4 weeks prior to the start of the study; 2.Have a mental disorder; 3.Have a tumor and have received chemotherapy or radiation therapy in the last six months; 4.Had a heart attack; 5.Combined with other major diseases, the expected natural course of disease is less than 1 year; 6.Internet access is not possible at home; 7.Internet access is not possible at home;
