Inclusion criteria

Inclusion criteria: 1. All outpatients or emergency department patients with suspected Cardiorenal Metabolic Syndrome (CRS). 2. Aged 18 years or older.

Exclusion criteria

Exclusion criteria: 1. Patients with severe cognitive impairment or inability to communicate. 2. Patients with a clear referral to a specific department (e.g., some patients transferred via emergency medical services [EMS], who may directly access the green channel). 3. Patients with unstable vital signs. 4. Patients with potential doctor-patient disputes. 5. Patients currently participating in other clinical trials. 6. Patients unable to comply with the trial procedures. 7. Patients who refuse to sign the informed consent form.
