Inclusion criteria

Inclusion criteria: 1. patients who met the diagnostic criteria of "Chinese Guidelines for the Diagnosis and Treatment of Heart Failure 2024" and were clinically diagnosed with HF; 2. Social frailty index (SVI) score >=12.25; 3. New York Heart function class II-IV; 4. age >=60 years old; 5. Able to communicate in daily language and volunteered to participate in this study.

Exclusion criteria

Exclusion criteria: 1. After heart transplantation; 2. Combined with severe cognitive impairment (MMSE<=9), severe mental illness, severe liver and kidney dysfunction, malignant tumor; 3. Participating in other clinical studies.
