Inclusion criteria

Inclusion criteria: Inclusion Criteria for Family Doctor Teams: 1. The team must have been providing family doctor contract services at the institution for a minimum of =1 year. 2. The team leader must be a family doctor whose licensed scope of practice is General Medicine (Western Medicine). 3. The team leader must conduct general practice outpatient clinics for at least =2 sessions per week. Inclusion Criteria for Residents: 1. Age of the resident must be >=35 years. 2. Must have an established contract relationship with a Family Doctor Team included in this study and have been receiving contract services from that team for at least 1 year (Note: Residents under contract with other Family Doctor Teams not included in this study are temporarily excluded). 3. Must have completed their 2025 annual health examination at this institution according to the Basic Public Health Service Standards. 4. Must be a resident visiting the general practice outpatient clinic on the day of enrollment (this is not limited to residents under contract with the consulting doctor). 5. Must provide signed informed consent.

Exclusion criteria

Exclusion criteria: Exclusion Criteria for Family Doctor Teams: 1. The team leader practices as a Traditional Chinese Medicine (TCM) category General Practitioner (temporarily excluded from this study). 2. The primary location of the family doctor's general practice outpatient clinic is a branch hospital, sub-center, community health service station, or village clinic of the primary healthcare institution (temporarily excluded from this study). Family doctor teams identified for this study must meet the aforementioned inclusion and exclusion criteria. Exclusion Criteria for Residents: 1. Employees of this institution and their family members. 2. Residents seeking care outside the scheduled hours of the general practice outpatient clinic. 3. Residents requiring urgent referral. 4. Individuals with communication barriers preventing their cooperation. Residents identified for this study must meet the aforementioned inclusion and exclusion criteria, as well as the requirements for associated family doctor teams.
