Inclusion criteria
Inclusion criteria: 1.Aged 18 years or older. 2.On fully controlled, pressure-targeted mechanical ventilation, with no spontaneous respiratory effort. 3.Cardiogenic shock due to various causes (e.g., acute myocardial infarction, myocarditis, cardiomyopathy, or post–coronary artery bypass grafting), with the diagnosis meeting the 2025 definition/criteria. 4.Refractory to conventional therapy and planned for VA-ECMO support. The decision to initiate VA-ECMO is at the discretion of the attending physician; initiation criteria typically correspond to SCAI stage C or higher.
Exclusion criteria
Exclusion criteria: 1.Patients with spontaneous breathing. 2.Refusal to participate in the trial. 3.Poor echocardiographic imaging quality or intolerance to transthoracic echocardiography (TTE). 4.Inability to obtain echocardiographic data prior to VA-ECMO initiation. 5.Presence of a pacemaker and/or an intra-aortic balloon pump (IABP). 6.Valvular heart disease. 7.Atrial fibrillation occurring during the study period.
