Inclusion criteria

Inclusion criteria: 1. It complies with the diabetes diagnostic criteria stipulated in the "Chinese Guidelines for the Diagnosis and Treatment of Diabetes in the Elderly (2024 Edition)". 2. Age >=65 years old; 3. During the period of home quarantine after discharge, take more than or equal to 1 long-term prescription medication.

Exclusion criteria

Exclusion criteria: 1. Those with consciousness disorders, cognitive function disorders, or speech communication disorders; 2. Patients with acute and critical conditions complicated with DM; 3. Those who are hospitalized due to changes in their condition and continue to receive treatment; 4. Combined with malignant tumors, liver and kidney failure, and other severe diseases.
