Inclusion criteria

Inclusion criteria: 1. Community-dwelling adults aged >=60 years; 2. Individuals identified as frail, screened using the Tilburg Frailty Indicator (TFI) with a score >=4; 3. Participants who provide informed consent.

Exclusion criteria

Exclusion criteria: 1. Individuals with severe physical illnesses and/or terminal conditions; 2. Those with a history of neurological disorders or major psychiatric illnesses; 3. Individuals with severe visual/auditory impairments or mobility limitations; 4. Participants concurrently enrolled in other intervention studies; 5. Other reasons deemed by the general practitioner as unsuitable for participation in this study.
