Inclusion criteria

Inclusion criteria: Inclusion criteria for patients: 1. Age >=60 years old; 2. Meet the diagnostic criteria for stroke and have a clear diagnosis through imaging examinations; 3. Proficient in using wechat; 4. Possess normal communication and reading skills; 5. Informed consent and voluntary participation in the research. Caregiver inclusion criteria: 1. Age >=18 years old; 2. The patient's spouse, children, relatives and other family members; 3. The family member who takes care of the patient for the most time, or is identified by the patient as the main caregiver; 4. Proficient in using wechat; 5. Possess normal communication and reading skills; 6. Informed consent and voluntary participation in the research.

Exclusion criteria

Exclusion criteria: Patient exclusion criteria: 1. Accompanied by severe organ dysfunction or combined with malignant tumors; 2. There are mental disorders or cognitive impairments. Caregiver exclusion criteria 1. Care remuneration needs to be paid; 2. Have severe organ dysfunction or malignant tumors; 3. There are mental disorders or cognitive impairments.
