Intervention Group:Development of an individualized lifestyle management program (low-glycemic-index diet and exercise guidance) based on continuous glucose monitoring.
Control group:Standard community-based health education and follow-up management.

Inclusion criteria
Inclusion criteria: 1. Meets the blood-glucose criteria for prediabetes; 2. Aged 20–70 years; 3. Able to communicate normally and use a smartphone; 4. Willing to participate and has signed the informed consent form.
Exclusion criteria
Exclusion criteria: 1. Pregnant or lactating women; 2. Concurrent use of medications that affect glucose metabolism (e.g., glucocorticoids, thyroid hormones); 3. Presence of severe cardiac, hepatic, renal, pulmonary, cerebral, or psychiatric disorders, malignant tumors, tuberculosis, or other chronic wasting diseases; 4. History of severe infection, major surgery, or serious trauma within the past 3 months; 5. Any other condition that, in the opinion of the investigators, renders the participant unsuitable for the study.
