Inclusion criteria
Inclusion criteria: 1. Patients who meet the CKD diagnostic criteria according to KDOQI clinical guidelines; 2. Age >=18 years; 3. Patients who have not received renal replacement therapy; 4. Patients who are able to care for themselves; 5. Able to communicate normally, with certain reading and comprehension abilities; 6. Able to use a smartphone; 7. Informed about this study and voluntarily willing to participate.
Exclusion criteria
Exclusion criteria: 1. Patients who have undergone dialysis or kidney transplantation; 2. Patients who are unable to care for themselves; 3. Patients with cognitive impairment or mental disorders; 4. Patients with other severe diseases; 5. Patients with serious complications.
