Inclusion criteria

Inclusion criteria: Content One: People aged 60 and above who receive diabetes management in the community. Inclusion criteria: 1. Individuals aged 60 and above; 2. Those registered in the community chronic disease management system and receiving diabetes management before December 31, 2024. Content Two: People aged 60 and above with coexisting diabetes and tuberculosis. Inclusion criteria: 1. Permanent residents of Shanghai; 2. Individuals aged 60 and above; 3. Onset of tuberculosis after June 31, 2025; 4. No communication barriers (mental, visual, auditory or speech) and possess the skills to use the APP; 5. No family members in the same household have participated in the trial; 6. Agree to participate in this study and sign the informed consent form.

Exclusion criteria

Exclusion criteria: Exclusion criteria for content one: 1. Diabetes management subjects who have been transferred due to various reasons before January 1, 2015; 2. Diabetes management subjects without follow-up records during the period from 2015 to 2024. Exclusion criteria for content two: 1. Patients with other serious diseases or in the terminal stage of disease, who are unable to participate in long-term follow-up; 2. Patients who have received similar intervention measures or participated in other clinical trials within the past six months.
