Inclusion criteria
Inclusion criteria: 1.Patients with comorbidities aged 18 years and above who have been receiving long-term treatment at health centers/community health service centers in Pinghu City, Zhejiang Province, suffering from 2 or more chronic diseases, and using at least two or more medications.
Exclusion criteria
Exclusion criteria: 1.Acute disease patients, terminally ill patients, malignant tumor patients, dementia patients, and those who did not complete the questionnaire are excluded.
