Trial Group:An Integrated Model of Stratified Dynamic Management, Personalized Education, and Online Continuing Care
Control Group:For patients in the control group, nurses provide routine health education according to the original responsibility-based grouping, including bedside verbal education and random of healt
meanwhile, patients are closely monitored during dialysis, and ESA and/or iron supplements are administered as per medical instructions based on individual patient conditions.

Inclusion criteria

Inclusion criteria: 1. Age >=18 years 2. Maintenance hemodialysis (MHD) treatment for >=3 months (excluding interference from acute kidney injury) 3. Regular follow-up visits are conducted monthly based on historical data

Exclusion criteria

Exclusion criteria: 1. Combine active bleeding, malignant tumors, and hematological diseases 2. History of blood transfusion or ESA dose fluctuation >50% in the past 3 months (affecting anemia evaluation) 3. Mental disorders or cognitive disorders (MMSE<=24 points) 4. Plan to transfer to another hospital/undergo kidney transplantation within 3 months 5. Pregnant or lactating women 6. Participation in other intervention studies
