Inclusion criteria

Inclusion criteria: 1. Participants must be aged 40 years or older; 2. The time elapsed from discharge after stabilization following hospitalization for acute exacerbation of chronic obstructive pulmonary disease (COPD) to enrollment is less than 7 days; 3. Participants are prescribed a metered-dose inhaler or a dry powder inhaler (e.g., Easyhaler) for maintenance therapy at discharge; 4. The participant or their long-term, stable caregiver can use a WeChat mini-program for self-management after training.

Exclusion criteria

Exclusion criteria: 1. Failure to sign a written informed consent form; 2. Participants who are arranged for hospice care after discharge; 3. Participants who are currently enrolled in other clinical trials involving medications or pulmonary rehabilitation; 4. Participants with aphasia, dementia, or other conditions that prevent effective communication.
