60 Years to 100 Years

Inclusion criteria

Inclusion criteria: 1. Diagnosed with a rheumatic disease. 2. Complicated with at least one chronic disease. 3. Aged 60 years or older. 4. In a stable condition and able to cooperate with nursing assessment and intervention.

Exclusion criteria

Exclusion criteria: 1. Patients with acute attack of rheumatic diseases requiring emergency treatment. 2. Patients with malignant tumors or other end-stage diseases (with an expected survival period of less than 6 months).
