Inclusion criteria
Inclusion criteria: 1.Age >=55 years; 2.Patients who have experienced ischemic stroke for the first time in clinical practice and who have experienced ischemic stroke recently (within 3-6 months).(2) The diagnosis depends on CT or MRI examination of the head, and cerebral hemorrhage or non-ischemic nervous system diseases are excluded, and the diagnostic criteria of China Guidelines for Diagnosis and Treatment of Acute Ischemic Stroke are met; 3.Presence of one or more vascular risk factors such as hypertension, diabetes, dyslipidemia; 4.MRS score 0-3; 5.Patients or their family members have normal communication and cognitive skills, can use mobile phones, WeChat and other Internet tools;and are willing to sign informed consent forms.
Exclusion criteria
Exclusion criteria: 1.Radiological evidence suggests a cerebral hemorrhage; 2.A definite cause of stroke or transient ischemic attack unrelated to atherosclerosis (e.g. carotid dissection or perivascular procedural stroke); 3.Stroke due to severe cardiogenic embolism; 4.Serious comorbidities or unstable conditions (e.g. congestive heart failure, severe cardiogenic embolism, respiratory failure, renal failure, severe liver dysfunction, or malignancy that may die within 2 years); 5.Hemorrhagic stroke and fatal stroke were excluded; 6.Severe memory or behavior disorders requiring routine care.
