Inclusion criteria

Inclusion criteria: 1. Assessed using the Chinese version of the Kihon Checklist (KCL) for frailty; a total score exceeding 7 indicates a frailty state; 2. Presence of two or more CMM diseases (such as hypertension, type 2 diabetes, etc.); 3. Age >= 60 years; 4. Voluntarily signs the informed consent form; 5. Able to clearly express personal feelings and needs.

Exclusion criteria

Exclusion criteria: 1. Patients who only have frailty or CMM; 2. Patients with severe cardiovascular diseases, respiratory system diseases, liver or kidney dysfunction, or other complications that may affect the study results; 3. Patients with other severe psychiatric disorders besides frailty.
