Inclusion criteria

Inclusion criteria: 1. Age 60–100 years, ASA classification II–IV 2. Patients with a Frailty Assessment Score >=1 3. Patients undergoing elective painless ERCP in the ERCP Endoscopy Unit at Beijing Chaoyang Hospital

Exclusion criteria

Exclusion criteria: 1. History of epistaxis, propofol allergy, or contraindications to HFNC (relative contraindications: severe type I respiratory failure; ventilatory dysfunction; pH < 7.30); 2. Paradoxical breathing; 3. Poor airway protection with high risk of aspiration; 4. Haemodynamic instability requiring vasoactive medications; 5. Facial or upper airway surgery precluding HFNC use; 6. Severe nasal obstruction; intolerance to HFNC. 7. Absolute contraindications: cardiac arrest requiring emergency tracheal intubation and invasive mechanical ventilation; 8. Weak spontaneous breathing, coma; extremely severe type I respiratory failure; ventilatory dysfunction with pH < 7.25; inability or refusal to sign informed consent for the study.
