Inclusion criteria
Inclusion criteria: 1. Age between 60 and 84 years; 2. Educational level at primary school or higher; 3. Gait speed test result <=1.0 m/s; 4. History of sarcopenia and/or osteoporosis, or during screening: calf circumference <34 cm (males)/<33 cm (females) and grip strength <28 kg (males)/<18 kg (females), or ultrasound bone mineral density T-score <=-2.5; 5. Expected to live in their current residence for the next 18 months(not residing in nursing homes)); 6. Willingness to participate in the fall prevention program; 7. Having experienced at least one fall (regardless of injury) within the past 12 months. 8. Lack a regular exercise habit, or engage in moderate-intensity exercise for no more than 150 minutes per week.
Exclusion criteria
Exclusion criteria: 1. Being in the acute phase of an illness or the acute exacerbation phase of a chronic disease; 2. Having hearing or visual impairments, or self-reported mental illness that prevents normal communication, interaction, or learning; 3. Having other medical conditions unsuitable for exercise (as advised by a physician or determined on-site by the evaluator); 4. Illiterate or unable to complete basic reading.
