Inclusion criteria
Inclusion criteria: 1.Age >= 65 years, any gender. 2.Residing in a senior care facility, diagnosed with at least one chronic condition requiring long-term medication. 3.Willing to receive pharmaceutical care and able to provide signed informed consent by the participant or their legal guardian. 4.Absence of severe cognitive impairment, and able to cooperate with the pharmaceutical services and provide relevant information.
Exclusion criteria
Exclusion criteria: 1.Critically ill or terminally ill older adults. 2.Older adults with severe functional disability. 3.Individuals with communication barriers (including hearing, cognitive, or other psychiatric impairments).
