Inclusion criteria

Inclusion criteria: 1. Confirmed medical diagnosis of T2DM for a minimum duration of six months, verified through medical chart review; 2. Age between 30 and 59 years; 3. Recent Hemoglobin A1C (HbA1C) level greater than 7%; 4. Ownership of a smartphone and regular use of the WeChat application for daily communications; 5. Proficiency in speaking and reading Mandarin; 6. Expression of consent and willingness to participate in the program.

Exclusion criteria

Exclusion criteria: 1.Experiencing significant health complications that impede self-care activities, such as advanced-stage cancer, impending limb amputation, substantial visual impairment, severe stroke, or end-stage renal disease; 2.Documented history of psychiatric conditions that could affect participation or study outcomes; 3.Anticipated relocation to a different city within the ensuing six months, which could disrupt ongoing participation; 4.Current enrollment in an alternative diabetes education program, to prevent confounding effects on the intervention.
